Download and review the complete Onsite Review Instrument and Instructions (OSRI).

Shawntese Jordan Mock Case

Download and review the Shawntese Jordan foster care mock case. After you have reviewed the mock case, begin applying the OSRI by documenting factual information that is in the case record. Make notes of any other information found in the case file that may need to be confirmed by interviews. Also note any missing or contradictory information. Complete as much of the OSRI as you can before moving on to step 3.

Please note that the full mock case is approximately 200 pages long. You may also elect to download the mock case in individual sections.

Listen to the mock case interview audio files. Using the information from those and reconciling it with the information in the case files, continue and complete your mock case review by rating the items and outcomes and otherwise documenting your responses as instructed in the OSRI.

OMS REVIEW SITES: DESCRIPTION AND FORMS

 

CFSR OMS Review Sites (PDF)—This document provides information on the three purposes of the OMS—training, continuous quality improvement (CQI), and CFSR.

OMS Training Review Contact Form (Word)—This form should be used to provide JBS with the names and contact information of each person who should have access to each state’s OMS training review site.

OMS CQI Review Contact Form (Word)—This form should be used to provide JBS with the names and contact information for each state’s OMS CQI review site.

CFSR State Contact Form (Word)—This form should be used to provide JBS with the names and contact information for each state’s CFSR site.

CFSR Online Monitoring System User Manual

This manual provides an overview of the CFSR Online Monitoring System (OMS), step-by-step instructions for using the site, and a description of each role (Reviewer, State Site Leader, Federal Site Leader, State Administrator, and Observer).
