Onsite Review Instrument and Instructions

OSRI (available in English [PDF] and Spanish [PDF]): This instrument is used to review cases during the onsite review component of the CFSRs. Note that while this document cannot be completed electronically, it can be printed and completed by hand.

OSRI (fillable PDF): This is a format of the OSRI that can be completed electronically or printed and completed by hand. Note that if the document is completed electronically, information entered into text boxes may not print in its entirety on paper but remains available electronically. You may navigate the document using the Tab key or a mouse.

Quick Reference Items List (PDF): This document briefly summarizes the 18 items in the Onsite Review Instrument and Instructions and the 18 systemic factor items.

OSRI Revisions (PDF): This document describes changes made in January 2016 in the hard copy OSRI to match it with the corresponding changes in the Online Monitoring System (OMS).

Reviewer Brief - Understanding the Federal Expectations for Rating Cases (available in English [PDF] and Spanish [PDF]): This brief provides an overview for CFSR reviewers of the practices that may lead to a Strength rating in a case and the expectations or special considerations that should be given to particular case circumstances in evaluating case ratings.

Reviewer Brief - Strengthening How Reviewers Determine Effectiveness of Agencies' Assessment of Underlying Needs and Provision of Services (OSRI Item 12) (PDF): This brief is intended to strengthen how CFSR Reviewers determine, during case reviews, whether or not the agency (1) made concerted efforts to assess the needs of children, parents, and foster parents to identify the services necessary to achieve case goals and adequately address the issues relevant to the agency's involvement with the family, and (2) provided the appropriate services. It builds upon instructions provided in the Reviewer Brief above (Understanding the Federal Expectations for Rating Cases), and draws on information in the field of child welfare/social work regarding needs-based assessments and service plans.

Case Elimination Worksheet (Excel): States use this worksheet to list cases to be eliminated from the review sample, and the reasons for elimination.

Talking Points: Application of the OSRI to Cases Open as Both IH and FC During the PUR (PDF): This document provides guidance to CFSR participants reviewing a case that was opened for both in-home services and foster care during the period under review.

  

Statewide Assessment Instrument

Statewide Assessment Instrument (available in English [fillable PDF and nonfillable Word] and Spanish [fillable PDF and nonfillable Word]): This instrument template’s four sections are designed to enable states to gather and document information that is critical to analyzing their capacity and performance during the statewide assessment phase of the CFSR process. This template is offered in Adobe fillable PDF and Microsoft Word nonfillable formats. The Word version provides formatting flexibility. We encourage the state to use this document "as is" to complete the assessment, but the state may use another format as long as the state provides all required content.

  

CFSR Procedures Manual

CFSR Procedures Manual  (PDF): This manual describes the structure and process of the CFSRs, including the overall framework, the statewide assessment, the onsite review, case sampling, stakeholder interviews, determinations of substantial conformity, collaborating during the CFSRs, statewide data indicators, and the planning conference call schedule. Updated November 2015.

  

Stakeholder Interview Guide

SIG (PDF): This instrument is used to conduct local and state-level interviews during the onsite component of the CFSRs. It identifies questions that may be asked during stakeholder interviews across seven systemic factor sections.

Supplemental for the SIG (PDF): This supplemental provides alternative language and phrases that interviewers can apply to all the Stakeholder Interview Guide questions, if needed.

Guidance on Potential Data and Information That Can Be Used To Assess Systemic Factor Functioning (available in English [PDF] and Spanish [PDF]): This document provides guidance and examples to assist states in providing relevant data to evaluate systemic factor functioning pursuant to the Child and Family Services Plan and the CFSR statewide assessment.

A Guide for Implementing Improvement Through the CFSP and CFSR (PDF): This guide translates principles of effective implementation for states and the Children’s Bureau to use when working jointly on the development and implementation of the Child and Family Services Plan and the Child and Family Services Review Program Improvement Plan.

CFSR Technical Bulletin #9 (Transmittal Letter) (PDF): This cover letter transmits CFSR Technical Bulletin #9 (link below). It also includes the "List of CFSR Round 3 Statewide Data Indicators Issues as of Publication."

CFSR Technical Bulletin #9 (PDF): This Technical Bulletin (TB) rescinds TB#8A and establishes a new plan for use of statewide data indicators and national standards as context data for the third round of the Child and Family Services Reviews (CFSRs). The new TB reissues and revises applicable guidance formerly from TB#8A on program improvement plan measurement methods to reflect the change in the use of the statewide data indicators.

CFSR Technical Bulletin #8A (Transmittal Letter) (PDF): This cover letter transmitted CFSR Technical Bulletin #8A (link below), which has been rescinded by TB #9, above.

CFSR Technical Bulletin #8A, Amended (Final) (PDF): This TB has been rescinded by TB #9, above. TB #8A accompanied the amended (May 13, 2015) Final Notice of Statewide Data Indicators and National Standards for the CFSRs published in the Federal Register (link below). This information was specific to the third round of the CFSRs, and provided technical detail on how the Children's Bureau calculated whether a state had met national standards and on establishing program improvement goals relative to the statewide data indicators for states not meeting the national standards.

CFSR Technical Bulletin #7 (Cover Letter) (PDF): This cover letter transmitted CFSR Technical Bulletin 7, which announced the third round of the CFSRs.

CFSR Technical Bulletin #7 (Final) (PDF): Technical Bulletin #7 announced the third round of the CFSRs. It described changes to the review process and provided information on scheduling.

Data Indicators and National Standards

Statewide Data Indicators for Child and Family Services Reviews: Request for Public Comment (Link): This is the 2018 Federal Register notice requesting public comment on the revised syntax used to calculate statewide data indicators for the CFSRs. Before the Children's Bureau finalizes the revised syntax, and publishes revised performance results for all states and revised national standards, the agency invites state child welfare agencies, partner organizations, and the public to review, test, and provide comments on the revised syntax used to perform data quality checks and calculate observed performance on the statewide data indicators. Comments are due by February 25, 2019, and should be submitted via one of the methods provided in the notice.

Update: Use of CFSR Statewide Data Indicators in Round 3 - CB State Call Presentation (Transcript, PDF of Slide Presentation, WebEx): This is the Children's Bureau presentation in October 2016 on the CFSR statewide data indicators and national standards in conjunction with Technical Bulletin #9 (TB#9 and Transmittal Letter linked above). The Children's Bureau delivered this presentation during a WebEx briefing for states. The Administration for Children and Families' decision for Round 3, due to revisions, is to use the data indicators for context information, and to continue accountability for outcomes based on case review findings. All other aspects of the review process remain in place.

CFSR Round 3 Statewide Data Indicators - CB State Call Presentation (Transcript, PDF of Slide Presentation, WebEx): See important update in the later presentation, above. This is the earlier Children's Bureau presentation on the CFSR statewide data indicators and national standards with changes to the Federal Register notice issued in May 2015. The Children's Bureau delivered this presentation on June 25, 2015, during a WebEx briefing for states. The transcript and video include questions and answers. The transcript clarifies one answer (see page 15) given in response to a question asked during the presentation.

Executive Summary: Final Notice of Statewide Data Indicators and National Standards for Child and Family Services Reviews (PDF): This document summarizes the Children's Bureau's plan, published in the Federal Register on October 10, 2014, and amended on May 13, 2015, to replace the statewide data indicators and the methods for calculating associated national standards.

Federal Register Notice: Statewide Data Indicators and National Standards for Child and Family Services Reviews (link): This Federal Register Notice pertaining to the statewide data indicators and national standards for the third round of the CFSRs was published on May 13, 2015, providing corrected information to the Final Notice published on October 10, 2014 (earlier uncorrected notice: link).

Combined Federal Register Notice: Statewide Data Indicators and National Standards for Child and Family Services Reviews (PDF): A Federal Register Notice pertaining to the statewide data indicators and national standards for the third round of the CFSRs was published on May 13, 2015, providing corrected information to the Final Notice published on October 10, 2014. This combined version integrates the corrections into the original publication, so that all information pertaining to the data indicators is in one document. The corrected information is highlighted in the document. Other edits were made in this compilation to aid the reader in understanding the revised content in context.

CFSR Round 3 Statewide Data Indicator Series (link): These seven publications from the Capacity Building Center for States provide information and data on the seven CFSR indicators for use in continuous quality improvement (CQI) activities and CFSR trainings. These factsheets take complex data calculations related to states’ performances on CFSRs and condense them into a digestible reference to help state leaders, CQI staff, and other interested child welfare professionals understand and confidently explain CFSR data indicators to a variety of audiences.

CFSR Round 3 Findings

CFSR Aggregate Report, Round 3: FYs 2015-2016 (PDF): This report presents key findings from the analyses of state performance data for the states reviewed during the first 2 years of Round 3 of the CFSRs. The report describes results of a preliminary examination of the strengths and areas needing improvement identified across those states.

CFSR Round 3 Findings 2015-2016 (PDF): This PDF consists of 115 slides based on a Children's Bureau presentation of results from 24 states that participated in the FY 2015-2016 CFSRs. In those reviews, 2,073 cases were reviewed, including 9 Traditional Reviews that examined 584 cases and 15 State Conducted Case Reviews that examined 1,489 cases.

Instruments and Guides

Procedures Manual (Link) The Child and Family Services Reviews Procedures Manual offers an overview of the purpose and structure of the reviews, as well as detailed information on planning for and conducting the reviews.  The manual also discusses the Final Report and the Program Improvement Plan process.  It is designed to assist Children’s Bureau staff and state child welfare agencies in planning for, and participating in, a CFSR.  State agency administrators are strongly encouraged to share the manual with agency staff who will play active roles in the state’s CFSR, including Local Site Coordinators.

Highlights from the Procedures Manual:

  • Chapter 4: Sampling Guidelines: Provide information to states about the selection of a random sample of cases to be reviewed during the onsite review.  This document provides a summary of the sampling procedures.
  • Appendix C: Timeframes for the CFSRs: Include descriptions of review activities from 9 months to 5 years after an onsite review.
  • Appendix E: Tips on Creating Onsite Review Schedules: Provide guidance to states for creating the schedules for the following review team members at all three sites: Team Leaders, Local Site Leaders, and Reviewers.
  • Appendix G: Preparation for Interviews and Sample Interview Confirmation Letter

Statewide Assessment Instrument (PDF): A tool for states to use in examining their capacity and performance in improving outcomes for children and families engaged in child welfare services.  Each section of the instrument is designed to enable states to gather and document information that is critical to analyzing that capacity and performance during the statewide assessment phase of the CFSR process.

Onsite Review Instrument (PDF): Used to review both foster care and in-home services cases during the onsite review.  The instrument was automated for the second round of reviews; the automated version is based on the hard copy.

Changes to the Instruments (Word): Provides a summary of the major substantive changes to the statewide assessment, onsite review instrument, and stakeholder interview guide made since the first round of reviews.

Stakeholder Interview Guide (PDF): Used to conduct local and state-level interviews during the onsite review.  The purpose of the stakeholder interviews is to collect information for evaluating and rating the outcomes and systemic factors that are examined during the CFSRs.  The instrument was automated for the second round of reviews; the automated version is based on the hard copy.

Program Improvement Plan Matrix (PDF) The purpose of this document is to provide guidance to states on developing Program Improvement Plans in response to the CFSR findings.  This guidance, which originally was produced in 2002 for the first round of CFSRs, was updated for use during the second round of reviews.

Program Improvement Plan Annual/Quarterly Status Report Form:
This form is designed to be used by Children's Bureau Regional Office staff to evaluate Program Improvement Plan progress for states that are not using the new matrix in developing their Program Improvement Plans.  For states that are using the new matrix, Regional Offices should use
the new matrix to evaluate state Program Improvement Plans.  The new matrix is available
through Information Memorandum 07-08.

Conference Call Guidelines (Word): These provide information on scheduling and facilitating the planning and debriefing calls, including detailed proposed agendas for the calls.

Court Information Template (Word): Provides a template for collecting court-related information.

Case Elimination Worksheet (Excel): Used by states to inform the Children’s Bureau Regional Office of cases that have been eliminated from the CFSR onsite review sample.

Fostering Conections to Success and Increasing Adoptions Act of 2008

The “Fostering Connections to Success and Increasing Adoptions Act of 2008” legislation amends parts B and E of title IV of the Social Security Act to connect and support relative caregivers, improve outcomes for children in foster care, provide for Tribal foster care and adoption access, improve incentives for adoption, and for other purposes.  This page includes information from the Children’s Bureau about the legislation and guidance on its provisions.

Children and Families Having a Choice and a Voice

This documentary, produced by JBS and the Georgetown University Center for Child and Human Development's National Technical Assistance Center for Children's Mental Health, provides viewers with a clear picture of the impact home and community-based services are having on families and children.  In 2005, Congress authorized a 5-year demonstration grant program to test whether children with serious emotional disturbances who meet the requirements to receive services from a psychiatric residential treatment facility could be successfully served in a cost-effective manner with their families in the community.  The Centers for Medicare and Medicaid Services selected nine states to participate in this demonstration.  They ultimately provided community alternatives to residential treatment services to more than four thousand children.  The demonstration successfully enabled most of the children to maintain or improve their functional status while in the program at less than a third of the cost of serving them in an institution.

The video includes basic trauma information and lessons learned.  JBS is working with Georgetown University on the 5-year program, which is formally titled the Community Alternatives to Psychiatric Residential Treatment Facilities Demonstration Grant Program.  Section 6063 of the Deficit Reduction Act of 2005 authorized up to $218 million for the demonstration, from fiscal years 2007 through 2011.  A second video is planned to address trauma-informed practices.
